Providing short or long-term care to adults and the seniors
Plaza Health Network is there to serve adults and seniors who need nursing care, rehabilitation, day-to-day help with assisted living or palliative care. We also offer respite care – full time care for a few days or a few weeks – so that caregivers can rest and refresh themselves.
Short-term or transitional nursing and medical care
After surgery, injury or illness, many patients require short term nursing and restorative care as they transition from hospital to home. Our in-patient nursing and restorative care helps patients heal faster and avoid complications which can arise when patients return directly home following hospital treatment. Our care is designed to ensure patients are healthy enough to function safely and independently once they return home.

Rehabilitative care (short-term and long-term)
Our rehabilitation programs are designed to meet the needs of both short-term and long-term patients. Our aim is to help patients function at the highest possible level by providing the best possible treatment and therapy programs, getting them back home as quickly as possible, whenever possible.
Treatment by highly trained therapists and access to state-of-the-art equipment helps our patients get back on their feet, even in cases where patients who have been bed-ridden or wheelchair-bound for years.
Rehabilitation programs include post-operative rehabilitation, fall prevention and balance therapy, stroke therapy with the new Tibion Bionic Leg, comprehensive physical therapy, occupational therapy and speech therapy for patients following surgery, injury or illness.

Adult and senior long term care (assisted living, respite and palliative care)
We are committed to making all our patients feel at home when they are at our centers. Family members are always welcome at our centers and are encouraged to visit day or night, or take family members home for short term or extended visits. Families play an important role in our patients’ recovery and care and your active participation in their daily lives is always welcome and encouraged.
We provide long-term care for adults and seniors who need assistance with day-to-day living; respite care so that family members or caregivers can rest and rejuvenate, and palliative care that provides patients with comfort, dignity and respect.
