Plaza Health Network launches advanced respiratory care program at Arch Plaza

aAdvanced Respiratory CareAs part of its ongoing mission to provide high quality, sub-acute care to adults and seniors in the South Florida community, Plaza Health Network has launched an advanced, inpatient respiratory care program that provides treatment, rehabilitation and education to patients with complex respiratory conditions. The program is available at Arch Plaza, located in North Miami, and will continue to roll out at the other six Plaza Health Network centers throughout Miami-Dade County over the course of 2018.

Patients with complex respiratory conditions -such as chronic obstructive pulmonary disease (COPD), pneumonia, lung disease and those who are ventilator or tracheostomy dependent- will be cared for by respiratory therapists who coordinate with nursing and rehabilitation staff to provide enhanced and advanced pulmonary care. The team also is focused the patient’s continual care once they have transitioned from hospital to home by teaching them the necessary skills that would enable them to function independently at home, thereby avoiding unnecessary hospital readmissions.

Plaza Health Network and Jackson Health System have collaborated to provide clinically complex patients with an option for sub-acute care recovery.

Patients who require extra care and observation but do not have to be in a hospital setting can continue their recovery at one of Plaza Health Network’s seven rehabilitation and skilled nursing facilities. The relationship commenced between Jackson Memorial Hospital (JMH) and The Miami Transplant Institute (MTI) with Plaza Health Network’s two facilities in the Miami Health District, University Plaza and Jackson Plaza. These two facilities are in close proximity to the hospitals, providing JMH and MTI doctors and nurses ease of visitation if necessary to monitor their patients. Left ventricular assist device (LVAD) patients are among the first to benefit from this unique arrangement.

Medicaid expansion necessary

4-Ron Lowy Photo 2As chairman of the board of the largest network of not-for-profit rehabilitation and skilled nursing centers in South Florida, I applaud the Herald for its coverage of and the Editorial Board’s spotlight on the glaring need for Medicaid expansion.

Prompted by the recent report released by Florida Community Health Action Information Network, the Herald is again shining a bright light on this dire situation: One in four Miami-area adults are in poor health, and one in three working adults have no health coverage even with the new coverage options available through the Affordable Care Act. In Florida, there are 2.8 million uninsured individuals.

Mover’s Spotlight: Ann-Lynn Denker

Ann-Lynn-Denker-120x150New Position: Chief clinical officer for Plaza Health Network.
From: Jackson Health System, where she most recently was director of the Center for Nursing Excellence.

Also: Denker serves on the Board of the Health Council of South Florida. She also is the director of the Florida Action Coalition (Future of Nursing) statewide grant sponsored by the Robert Wood Johnson Foundation and Florida Blue. A former president of the Florida Nurses Association, Denker also served for five years on the Florida Board of Nursing and is immediate past chair. She is a longstanding member of the bioethics committee at Jackson Health System and currently teaches graduate students at the Barry University School of Nursing.

Education: Denker, an advanced register nurse practitioner, has B.S.N. and M.N. degrees from the University of Florida and received a Ph.D in nursing from the University of Miami.

Best advice she ever received: I have been most fortunate to have wonderful mentors who have provided valuable advice. The most far reaching advice has come from a nursing consultant colleague who convinced me of the importance of the practice of “Relationship Based Care.” This is a formalized culture for healthcare organizations where relationship with patient, families, colleagues, staff and ourselves are essential to quality patient care. It’s all about relationships, reflecting our core business of caring.
